To register a vehicle in the province of Alberta, proof of ownership is required at the time of registering.
This application must be approved by a certified medical professional and presented to a registry within 6 months. This application is required for either a first time application or a renewal of a parking placard.
Further testing by a vision specialist is required should you fail to pass the vision standards for the operator class you hold, either in office or with your medical practitioner.
When the owner(s) of a vehicle are unable to apply in person for vehicle registration services, this authorization form may be used.
If you are unable to apply in person for your driver’s abstract, these forms can be used to authorize another individual on your behalf. Both of these forms must be presented in office, by the person authorized.
To apply for access to motor vehicle information such as an employer obtaining driver abstracts for their employees, this application will need to be completed.
